CLHC OPERATES THROUGH COMMUNITY-BASED CENTERS, LOCATED IN MANHATTANS CHINATOWN, HARLEM and OTHER MEDICALLY-UNDERSERVED NIEGHBORHOODS THROUGHOUT THE NEW YORK CITY METROPOLITAN AREA WHICH ACT AS HUBS of WIDE RANGING SOCIO-MEDIVAL SERVICES INCLUDING HOME and ADULT DAY CARE, PRIMARY and EMERGENCY CARE, NURSING, SOCIAL WORK and THERAPY AS WELL AS TRANSPORTATION and INPATIENT SERVICES. CLIENTS ARE LINKED to HIGH QUALITY MEDICAL CARE THROUGH DEDICATED STAFF MEMBERS WHO SEEK to MAINTAIN EACH CLIENTS HIGHEST LEVEL of FUNCTIONING and WELL-BEING.
