THE VARIOUS CLINICAL PROGRAMS ARE CO-LOCATED WITHIN FACILITIES to ENSURE THE INTEGRATION of ALL NEEDED CARE for PERSONS WITH MULTIPLE DISORDERS. SERVICE PLANS ARE BASED ON INDIVIDUALIZED NEEDS and CHOICES RATHER THAN FIXED PROGRAM DESIGNS.
