JOSEPHS HOSPITAL and MEDICAL CENTER and ITS AFFILIATES, PROVIDED THAT THEY OPERATE EXCLUSIVELY for CHARITABLE PURPOSES WITHIN THE MEANING of SECTION 501(C)(3) of THE CODE. EFFECTIVE JUNE 30, 2017 ASSETS WERE TRANSFERRED to DIGNITY HEALTH and ST. JOSEPHS FOUNDATION, A SUPPORTING ORGANIZATION of DIGNITY HEALTH DBA ST. JOSEPHS HOSPITAL and MEDICAL CENTER. THE CONGENITAL HEART FOUNDATION WILL BE DISSOLVED AS A CORPORATION IN FISCAL YEAR 2018.
