FUNDING ASSISTS INDIVIDUALS THAT ARE MEDICALLY FRAIL or DEVELOPMENTALLY DISABLED THAT LIVE AT HOME WITH THEIR PARENTS AS WELL AS THOSE THAT LIVE IN ONE of OUR MEDICAL HOMES.
