CHC SERVES A NINE-COUNTY AREA IN NORTHERN INDIANA, INCLUDING ELKHART, FULTON, KOSCIUSKO, LAGRANGE, LA PORTE, MARSHALL, ST. JOSEPH, STARKE, and PORTER COUNTIES. CHCS MISSION is to IMPROVE THE QUALITY of LIVING THROUGHOUT NORTHERN INDIANA, and OUR VISION is to BE THE PREMIERE HOSPICE and PALLIATIVE CARE ORGANIZATION for ALL END-OF-LIFE ISSUES.
