COLLABORATES ON COMMUNITY EVENTS WITH KYHH PARTNERS, PROVIDES REFERRALS to SOCIAL SUPPORT SERVICES SUCH AS HOUSING and CEDA PROGRAMS. PROVIDES CLASSES ON NUTRITION, DIABETES and CVD PREVENTION EDUCATION.WORK WITH LOCAL PARTNER ORGANIZATIONS, BUSINESSES and HEALTH AGENCIES to IDENTIFY and PROVIDE RESOURCES THAT WILL SUPPORT FOLLOW UP EFFORTS WITH COMMUNITY RESIDENTS.
