These Included Individualized Case Management, Transportation Assistance, Job Readiness Training, and Connections to Mental Health, Substance Use, and Medical Providers. Services Focused On Removing Barriers to Stability and Increasing Self-sufficiency. Over 90% of Participants Engaged In At Least One Support Service Per Week.
