ABCD ALSO DEVELOPED A STRATEGIC PLAN, EVALUATION PACKAGE, AND MONTHLY COMMUNICATION PACKAGES FOR A CDC PROJECT, HEALTHY TIES THAT BIND, FOR THE LEADERSHIP COUNCIL FOR HEALTHY COMMUNITIES GRANT FROM CENTER FOR DISEASE CONTROL AND PREVENTION. THIS WORK FOR LCHC CONNECTS THE RESIDENTS OF WARDS 4, 5, 7, AND 8 WITH CLINICAL AND COMMUNITY PREVENTIVE HEALTH SERVICES TO SELF MANAGE THEIR DIABETES, HYPERTENSION, AND HEART DISEASE.
