THE PURPOSE of THIS ORGANIZATION is to PROVIDE EDUCATION and SUPPORTIVE SERVICE THAT WILL STRENGTHEN FAMILIES, ORGANIZATIONS and AT-RISK INDIVIDUALS IN EFFORTS to CULTIVATE EXTRAORDINARY HEALTH OUTCOMES and BRIDGE THE GAPS BETWEEN COMMUNITY and HEALTHCARE SYSTEMS. THE ASSOCIATION SHALL FULFILL ITS PURPOSE BY: -GATHERING, ANALYZE, and DISSEMINATING DATA THAT PERTAINS to MORTALITY and MORBIDITIES. -DEVELOP, SUSTAIN, and DELIVER COST-EFFECTIVE, HIGH-QUALITY HOME and COMMUNITY HEALTH SERVICES. -REDUCE BARRIERS THAT PREVENT COMMUNITY MEMBERS FROM ACCESSING HOME and COMMUNITY-BASED HEALTHCARE SERVICES. -PROVIDE HEALTHCARE EDUCATION and SUPPORTIVE SERVICES for COMMUNITY MEMBERS and ORGANIZATIONS WITHIN THE COMMUNITY.
