DURING 2024/2025, COMMUNITY NURSE CONNECTION PARTNERED WITH THE TOWNS of PUTNEY, WESTMINSTER, and SANDWICH to SUCCESSFULLY HIRE COMMUNITY NURSES. EACH COMMUNITY WAS AWARDED A GRANT of 5,000. WE PROUDLY SUPPORT COMMUNITY NURSES IN A GROWING NUMBER of TOWNS ACROSS THE UPPER VALLEY of NEW HAMPSHIRE and VERMONT. BEGINNING IN THE SUMMER of 2024, COMMUNITY NURSE CONNECTION JOINED A COALITION of LOCAL PARTNERS to OFFER MULTI-SERVICE "POP-UP CLINICS" PROVIDING BASIC MEDICAL CARE and SUPPORT for BASIC and SOCIAL NEEDS. THESE CLINICS ARE HELD AT THE LEBANON WINTER SHELTER and THE KILTON PUBLIC LIBRARY SO AS to DELIVER ACCESSIBLE, COMPASSIONATE, and ESSENTIAL SERVICES to UNHOUSED NEIGHBORS IN OUR COMMUNITY. IN THE CURRENT YEAR, COMMUNITY NURSES SERVED 570 CLIENTS WITH NEEDS INCLUDING IMPAIRED MOBILITY, SYMPTOM MANAGEMENT, MEDICATION MANAGEMENT, MENTAL HEALTH and COGNITIVE IMPAIRMENT, FINANCIAL STRESS, and TRANSPORTATION.
