ESSENTIAL BEHAVIORAL HEALTH SERVICES INCLUDE: SCREENINGS, INDIVIDUAL, GROUP and FAMILY THERAPEUTIC SERVICES, COMMUNITY RESOURCE COORDINATION, OUTREACH, SHORT-TERM RESIDENTIAL CRISIS STABILIZATION, PSYCHIATRY, 24/7 CRISIS ASSESSMENT SERVICES, VOCATIONAL SERVICES, SUBSTANCE USE DISORDER SERVICES, and DUAL DIAGNOSIS SERVICES for PATIENTS WITH COOCCURRING MENTAL HEALTH and SUBSTANCE USE DISORDERS. PRIMARY CARE INTEGRATION INCLUDES BEHAVIORAL and ORAL HEALTH ACROSS ALL AGES USING POPULATION BASED SCREENING INCLUDING SCREENING for SOCIAL DETERMINANTS of HEALTH, DEPRESSION and ANXIETY.
