Therefore, The FAITHworks Health and Wellness Institute (SFW), A Separate, 501(c)(3) Nonprofit Medical Organization Dedicated to The Delivery of The SMART Models Clinical Care and Standards Was Organized and Legally Formed In Spring of 2022, and Through The 1023 Filing, Later Received It Full IRS Credentialing Status. It is Managed By An Independent Board of Directors, Which Includes Christopher Conti, M.D., Who Serves As President of The Organization and Who Oversees All Clinical Operations.In August of 2022, SFW Opened Its First Two SMART Clinics, At The LIFE Male STEAM Academy, A Public Charter School Primarily Serving African American Males, and The Calcutt M
