Our Permanent Supportive Housing PSH Program Provides Long-term Case Management and Supportive Services for Adults Who Have Experienced Chronic Homelessness and Face Significant Barriers Such As Mental Health Conditions Physical Disabilities or Substance Use Recovery. While Participants Rent is Covered Through External Housing Programs or Subsidiesnot By Our Organizationwe Play A Vital Role In Supporting Their Success In Housing Through Individualized Case Management.Services Include Life Skills Coaching Goal Planning Resource Navigation and Emotional Support All Delivered With A Trauma-informed Person-centered Approach. By Addressing The Underlying Factors That Contribute to Housing Instability We Help Participants Maintain Their Housing Improve Their Overall Well-being and Build Stronger Connections Within Their Communities. Our Team Collaborates Closely With Local Service Providers to Ensure Wraparound Support and Lasting Stability. In 2024 This Program Served 28 Households. Total P
