Collective Roots APG Provides Peer-based Recovery Support for Teens and Their Families Affected By Substance Use Disorders. Through Group Meetings Individual Support Pro-social Activities and Crisis Planning We Foster A Safe Substance-free Environment for Recovery. In The Past Year Our APG Served Approximately 20 Teens and Their Families Offering Ongoing Engagement and Continuity of Care. The Program Emphasizes Family Involvement and 100% of Participating Families Received Services On A Sliding Scale to Ensure Accessibility. We Also Provided Harm-reduction Tools Like Naloxone and Tailored Services for Youth With Co-occurring Mental Health Disorders.
