Social Work Advocates Primary Program is Connecting Aging Adults and Their Support Systems to Resources That Provide Stable Care As They Age Gracefully. SWA Connects Clients to Medicaid Resources, Mental Health Resources and Social Activities and Health Resources. In 2024 The Organization Established Relationships With Resource Partners and Began Meeting At Senior Facilities Hosting Social Events. The Events Were Set Up to Allow Aging Seniors Connection With Others, Provide An Opportunity for Exercise and Physical Activity While Gathering Input On Future Classes. SWA Also Spent Time Meeting At A Food Pantry to Assist Low-to-moderate Income Individuals and The Aging Community With Understanding Their Health Care Options and Assist Them With Getting Enrolled In Health Plans.
